    8 year-old boy with left-sided stomach ache, obvious pain on pressure in the left lower quadrant, the outer inguinal ring and spermatic cord inconspicuous, local pain on pressure of the left hemiscrotum.  

     
     Pediatric Radiology CasesPathomorphology or Pathophysiology of this disease :

    Pre-existing testicular separation with transversally positioned testicles ("mature turning testicles").  

     
     Pediatric Radiology CasesRadiological findings:


    Ultrasound 1 <- view Ultrasound 1

    Ultrasound 1: Depiction of the testicles and epididymis of both sides as a comparison. Obvious noticeable separation of the testicles and epididymis of the left side (and concurrent transversal, enlarged left testicle). Concomitant hydrocele.




    Ultrasound 2 <- view Ultrasound 2

    Ultrasound 2: Color doppler shows vascularization in the epididymis but not in the testicle.





    OP-Situs 1 <- view OP-Situs 1

    OP-Situs 1: Mesorchial testicular torsion. Hemorrhagic infarction of the testicle by well-vasculated epididymis and spermatic cord.





    OP-Situs 2 <- view OP-Situs 2

    OP-Situs 2: Noticeable separation of testicles and epididymis, transversal testicular position.

          Sehr geehrter Herr Kollege,
          entschuldigen Sie die späte Antwort, durch meinen Urlaub war ich verhindert.
          Dieser Fall ist sonographisch und von der intraoperativen Pathologie sehr interressant. Von der Anamnese her jedoch wenig auffällig, was die Torsion angeht.
          Der Junge klagte über plötzliche Schmerzen im linken Unterbauch. Initial kam es zu einmaligem Erbrechen. Bis zur Vorstellung vergingen ca. 10 Stunden. Auch jetzt stand der Unterbauchschmerz im Vordergrund. Bei der klinischen Untersuchung viel dann der druckschmerzhafte Hoden auf. Insgesamt war das Skrotum relativ gut zu untersuchen, so daß der querstehende Hoden zu tasten war. Die typischen (Spät-) Zeichen einer Torsion wie das Gersche Zeichen , das Tenckhoff Zeichen sowie das Prehnsche Zeichen waren nicht vorhanden.
