    Adolescent female patient, treatment resistant, long course of epilepsy with focal seizures. Focus was noticed in the right temporal area in the EEG.  

     
     Pediatric Radiology CasesPathomorphology or Pathophysiology of this disease :

    - Common cause of a temporal lobe epilepsy
    - Focal, most one-sided neuronal degeneration and astrogliosis with atrophy of the hippocampus, ammonic horn, dentate gyrus, subiculum and corpus amygdaoideum.  

     
     Pediatric Radiology CasesRadiological findings:


    MRI 1 <- view MRI 1

    MRI 1: T1-TIR: Examination in diagonal-coronal slicing (4 mm slice thickness): focal atrophy of the hippocampal region on the right side with asymmetrical temporal horn and thinning cortex of the hippocampus formation.




    MRI 2 <- view MRI 2

    MRT 2: T1-TIR, slight widened frontally located slice in diagonal-coronal slicing as in MRI 1: focal atrophy of the hippocampal region on the right side with asymmetrical temporal horn and thinning cortex of the hippocampus formation.




    MRI 3 <- view MRI 3

    MRI 3: T2-FLAIR-Sequence, the same slicing as in MRI 1 and 2: Increase in signals in the T2-weighted sequences: Astrogliosis with atrophy of the hippocampus, cornu ammonis, dentate gyrus, subiculum and corpus amygdaloideum.





    MRI 4 <- view MRI 4

    MRI 4: T2-TSE-Sequence, the same slicing as in MRI 1, 2 and 3: Increase in signals: Astrogliosis with atrophy of the hippocampus, cornu ammonis, dentate gyrus, subiculum and corpus amygdaloideum.

    MRI: Epilepsy due to hippocampal sclerosis

    Good pasitive case for TLE.My question,1.Many a times we encounter unilaterally small hippocampal head or body but without sinal alteration(astrogliosis).how do u interpret that?
    2.Moreover at times we come accross a Hippo.which is vertically orientated instead of horizontal, how do u expalin that?
