16 year-old boy with symptomatic splenic cyst (pain). History showed no evidence of trauma.  

Splenic cysts occur in echinococcus zysticus infections, congenitally or posttraumatically. In this case, the genesis (postraumatic or congenital) cannot be verified.

The most frequent cause for splenic cysts is infection with Echinococcus cysticus, followed by innate and post-traumatic cysts.

In this case, the cyst was punctured selectively using ultrasound. Subsequently, puncture and instillation of 96% alcohol solution as well as drainage by means of a Zystofix-catheter was performed. Initially, one saw an almost completely collapsed cyst; in the course of disease, however, reoccurrence, so that a hemisplenectomy of the lower part which contained the cyst was carried out. The cyst's contents and it's serology yielded no proof of the pathogen .  

In most cases of an echinococcus cyst, a preoperative echinococcus serology must be obtained. If the result is positive, a partial splenic resection should be done to the avoid pathogenic seeding into the peritoneal space; a preferential treatment when compared to marsupialization, for example.
