    11 year-old girl with a dorsal and a lateral swelling of the proximal humerus with pain on pressure and hard.  

     
     Pediatric Radiology CasesPathomorphology or Pathophysiology of this disease :

    An exostosis belongs to the benign bone tumors and is an osteoma which consists of a thorn-like part and an overlying cartilaginous cap. It arises from the bone surface, is mostly located in the long bones in proximity to the zones of growth, whithout touching the epiphyseal gap.

    X-Ray 1 <- view X-Ray 1

    X-ray 1: right Humerus, saggital: A stalked and a broad-based exostosis.



    X-Ray 2 <- view X-Ray 2

    X-ray 2: dorsal exostosis longitudinal.



    Ultrasound 1 <- view Ultrasound 1

    Ultrasound 1: Humerus, transversal: both exostoses (echogenic protrusion) with the above-lying hypoechoic cartilage cap.



    Ultrasound 2 <- view Ultrasound 2

    Ultrasound 2: Humerus, transversal: dorsal exostosis with measured cartilage cap.



    Ultrasound 3 <- view Ultrasound 3

    Ultrasound 3: Humerus, transversal: dorsal exostosis with measured cartilage cap.

    In a thick cartilaginous cap > 3 cm, a malignant illness must be ruled out.

    Seldom this becomes malignant.

    About 4 cm distally, proof of the vessels which cross humerus, whose brances correspond to the Brachii profunda arteries and veins. They are important as a guide to locate the radial nerve preoperatively.
